Richmar® Twin Stim® Plus 3rd Edition DS5402
The Richmar Twin Stim Plus DS5402 is a portable electrotherapy device with four channels that combines TENS, EMS/NMES, interferential current, and Russian stimulation in one compact system.
Four electrically isolated output channels and individual intensity controls let clinicians use up to eight electrodes, which provides broader coverage, bilateral placement, or treatment of multiple areas. The unit also includes two treatment timers, adjustable stimulation parameters, an LCD screen, battery or AC operation, and a patient-compliance meter for reviewing treatment-use records.
The DS5402 is particularly well suited to physical therapy clinics, chiropractic offices, rehabilitation departments, athletic-training rooms, sports-medicine programs, and professionally directed home treatment plans that need more electrode coverage than a standard two-channel device. Richmar identifies four therapeutic categories, five TENS programs, three EMS programs, three interferential programs, and three Russian programs.
Sale by or on the order of a physician or licensed practitioner. It should be used under the continuing supervision and instructions of a qualified healthcare professional.

How the four channels operate
The DS5402 has four isolated output channels, and every channel has separate intensity-increase and intensity-decrease buttons. One electrode pair may be connected to each channel.
However, four isolated channels do not mean four completely separate programmable treatments.
| Feature | Channel behavior |
|---|---|
| Intensity | Individually adjusted for CH1, CH2, CH3, and CH4 |
| Selected modality | Chosen through the device’s central Mode control |
| Program settings | Selected centrally for the active therapy mode |
| Left timer | Controls CH1 and CH2 |
| Right timer | Controls CH3 and CH4 |
| IF carrier pairing | CH1 pairs with CH2; CH3 pairs with CH4 |
| Maximum connected electrodes | Eight, using four electrode pairs |
Examples of four-channel flexibility
-
Four electrodes around one treatment region
-
Eight electrodes across a broader region
-
Symmetrical right-and-left placement
-
Two bilateral muscle groups
-
Two separate TENS regions using the same selected program
-
One four-electrode interferential field using CH1 and CH2
-
A second four-electrode interferential field using CH3 and CH4
Exact placement and channel selection must follow the prescription, device manual, and clinician’s treatment plan.
Therapy modes explained
| Modality | Intended response | Manual-listed treatment role |
|---|---|---|
| TENS | Strong but comfortable sensory stimulation | Symptomatic pain-relief programs and selected post-traumatic or postsurgical pain applications |
| EMS/NMES | Visible or palpable muscle contraction | Muscle re-education, spasm relaxation, disuse-atrophy programs, circulation and range-of-motion support |
| Interferential/IFC | Intersecting medium-frequency currents producing a beat frequency | Professionally directed pain, inflammation, and edema-related programs |
| Russian | Medium-frequency bursts producing structured muscle contractions | Muscle stimulation, re-education, spasm programs and strengthening-oriented contractions |
For a focused explanation of sensory versus motor stimulation, read TENS vs. NMES: Differences, Uses, and Device Guide.
TENS programs
| TENS program | How it operates |
|---|---|
| Burst | Groups nine 100 Hz pulses into adjustable bursts of 0.5–5 bursts per second |
| Normal | Provides continuous stimulation using the selected pulse rate and pulse width |
| Modulation | Automatically varies pulse rate and pulse width |
| SD1 | Modulates intensity and pulse width through a 40% range over a 10-second cycle |
| SD2 | Modulates intensity and pulse width through a 70% range over a 10-second cycle |
The adjustable TENS pulse-rate range is 2–150 Hz, and the pulse-width range is 50–300 µs.
EMS/NMES programs
| EMS program | Channel behavior | Adjustable controls |
|---|---|---|
| Constant | Continuous output | Rate, width, intensity and timer |
| Synchronous | Paired channels contract simultaneously | Ramp, on time, off time, rate, width and intensity |
| Alternate | Paired channels stimulate in sequence | Ramp, on time, off time, rate, width and intensity |
Interferential programs
The DS5402 creates interferential stimulation by pairing two channels with slightly different medium-frequency carriers.
| IF program | Frequency behavior |
|---|---|
| Normal | CH1 and CH3 remain at 4,000 Hz; CH2 and CH4 use a selected offset, producing a 1–150 Hz interference frequency |
| Modulation | Modulates through the lower 1–80 Hz difference-frequency range |
| Modulation 1 | Modulates through the higher 80–150 Hz range |
For one IF treatment field, Channels 1 and 2 supply four electrodes. Channels 3 and 4 can supply a second four-electrode field.
That distinction is important when comparing the DS5402 with the Richmar Quattro 2.5, which provides both IF-2P premodulated and IF-4P traditional interferential modes.
Russian programs
| Russian program | How it operates |
|---|---|
| Normal | Adjustable 20–80 Hz burst frequency with a fixed 50% duty cycle |
| Modulation | Automatically varies duty cycle between approximately 10% and 50% |
| Modulation 1 | Varies burst frequency between 20 Hz and the selected value |
All Russian modes use a 2,500 Hz carrier. Because these programs are meant to cause muscle contraction, they should be used under professional guidance, with proper electrode placement, work-rest timing, intensity, and monitoring.

Choose the Twin Stim Plus DS5402 when:
-
Four portable output channels are important.
-
The treatment plan requires TENS, EMS, interferential, and Russian stimulation.
-
Eight-electrode placement may be needed.
-
Four-electrode IFC is required.
-
AA batteries are preferred over a proprietary rechargeable battery.
-
AC operation is also useful.
-
Treatment-use tracking is important for a professionally directed home program.
-
A clinic needs a lower-cost portable alternative to a tabletop four-channel system.
Replacement electrodes
The DS5402 includes eight 50 × 50 mm, approximately 2-inch-square, self-adhering electrodes. Compatible replacements must accept the device’s lead-wire pins and be suitable for the selected modality.
| Replacement option | Best fit |
|---|---|
| Balego TENS/NMES/FES Electrodes | American-made everyday replacement pads in several sizes |
| InTENSity 2" Square Electrodes | Richmar-branded 2-inch-square white-cloth pads in a 40-electrode package |
| Classic TENS Electrodes | Soft foam, aloe hydrogel and silver/carbon construction |
| Starburst Electrodes | Premium silver-gradient current-distribution design |
| DURA-STICK PLUS Electrodes | Clinic-oriented replacement quantities |
| Electrotherapy Electrode Collection | Complete size, shape and brand comparison |
Use approximately the same electrode size originally selected or prescribed. A smaller electrode concentrates output over less skin and may feel sharper at the same setting.
Added conductive gel is normally unnecessary with pre-gelled self-adhering pads and may interfere with adhesion. Spectra 360 Electrode Gel and SignaGel Electrode Gel are intended for compatible non-gelled electrodes or procedure-specific applications—not routine application over reusable hydrogel pads.

Frequently asked questions
Is the Twin Stim Plus a TENS unit or an EMS unit?
It is a combination device providing TENS, EMS/NMES, interferential, and Russian stimulation.
Is it a prescription device?
Yes. It is used on the order of a physician or licensed practitioner.
Are all four channels independent?
The four outputs are electrically isolated, and each has separate intensity controls. The device uses one centrally selected mode and parameter set, while CH1–CH2 and CH3–CH4 have separate treatment timers.
How many electrodes can be connected?
Each channel accepts one electrode pair. Four active channels therefore use eight electrodes.
Can it provide two IFC treatments simultaneously?
The IF program pairs CH1 with CH2 and CH3 with CH4. This can support two separate four-electrode interferential fields when prescribed, correctly placed, and professionally supervised.
Does it provide two-pole premodulated interferential stimulation?
The DS5402 manual describes paired-channel interferential output, not a separate two-electrode premodulated mode. The Quattro 2.5 offers distinct IF-2P and IF-4P choices.
How many timers does it have?
It has two timers. The left timer controls CH1 and CH2, and the right timer controls CH3 and CH4. Each timer can be set for 1 to 60 minutes or for continuous operation.
Does it store treatment-use information?
Yes. The compliance meter stores 60 operation records and up to 999 accumulated treatment hours.
Is it rechargeable?
No. It operates on four replaceable AA batteries or an AC adapter.
Can rechargeable AA batteries be used?
Use only battery types permitted by the current device instructions. Do not attempt to recharge batteries through the DS5402.
What electrodes come with it?
The package includes eight reusable 50 × 50 mm self-adhering electrodes.
What replacement pads should I order?
The closest standard size is a 2-inch-square electrode with a compatible pin-style connection. Compare Balego electrodes and InTENSity 2" square electrodes.
Does it need conductive gel?
No, the included pre-gelled adhesive electrodes do not require additional gel. Added gel may reduce adhesion or contaminate their hydrogel.
Why does one channel feel weaker?
Possible causes include different electrode placement, varying skin resistance, a worn electrode, incomplete contact, a damaged lead, or a different channel-intensity setting. The manual recommends replacing dried or contaminated electrodes and worn lead wires.
Why does the output feel intermittent?
Burst, modulation, synchronous, alternate, and work-rest programs intentionally vary output. Unexpected interruption may instead indicate a loose connection, worn lead wire, lifted electrode, or low battery.
Can it be used at home?
It may be assigned for home use when prescribed and when the patient has been trained in mode selection, settings, electrode placement, contraindications, skin inspection, and safe operation.
What is the warranty?
One-year warranty.
