A step-by-step TENS pad placement guide for back, knee, and shoulder pain, including electrode setup, self-adhesive tips, and common mistakes to avoid.
Quick answer
Place TENS electrodes so the current crosses the painful area, with pads at least 2 inches apart and never over the front of the neck, temples, or broken skin. For back pain, straddle the spine; for knee pain, bracket the joint above and below; for shoulder pain, wrap the pads around the shoulder cap. Use self-adhesive electrodes with even hydrogel and press the edges down firmly before starting.
Prepare the skin and device before you place any pad
Clean the skin with mild soap and water, then dry it completely. Oils, lotion, and sweat weaken adhesion and can cause uneven current that feels sharp rather than soothing. If you have hair where the pad will sit, trim it rather than shave; shaving can irritate skin and make the pad lift sooner. Check that the TENS unit is off and the intensity dial is at zero before you connect anything. Plug the lead wires into the device first, then snap or clip them to the electrodes. This order prevents accidental stimulation while you handle the pads.
Back pain TENS placement: straddle the spine, not the bone
For low back pain, place one pair of electrodes on either side of the spine at the same height, about 2 to 4 inches apart. The current should cross the painful zone, not sit directly on the bony points of the spine. If the pain runs down into the buttock or leg, move the pair lower and slightly outward, but keep them on muscle rather than over the iliac crest. For mid-back pain, position the pads horizontally across the area, again avoiding the spine itself. A common mistake is placing both pads on the same side of the spine; that sends current along the muscle rather than across the pain site.
Knee pain TENS placement: bracket the joint above and below
For knee pain, place one electrode above the kneecap on the lower thigh and one below the kneecap on the upper calf, so the current crosses the joint. If the pain is on one side of the knee, place the pads on the inner and outer sides of the joint at the same level. Keep the pads off the kneecap itself; the bone has little soft tissue and current there can feel sharp. A checkpoint: when you turn the unit on, you should feel a gentle tingling across the knee, not a stinging pinpoint under one pad. If one pad stings, turn the unit off, press that pad down more firmly, and restart.
Shoulder TENS placement: wrap around the shoulder cap
For shoulder pain, place one electrode on the top of the shoulder and one on the back of the shoulder, so the current crosses the joint. If the pain is in the front of the shoulder, place the pads on the front and back of the shoulder cap. Avoid placing pads directly over the collarbone or the bony point of the shoulder. A common mistake is putting both pads on the upper arm; that misses the shoulder joint and often feels like a muscle twitch rather than relief. Check that the lead wire has slack so it does not pull the pad loose when you move your arm.
Electrode setup and self-adhesive tips that keep pads working
Self-adhesive electrodes rely on a thin hydrogel layer to conduct current evenly. Before each use, press the edges down with your fingertips for a few seconds. If the pad has lint or hair on the gel, rinse it with a drop of water and let it air-dry; do not wipe it with alcohol, which dries out the hydrogel. Store pads on their plastic liner in a sealed bag away from heat and sunlight. When a pad no longer sticks or you see the gel pulling away from the edges, replace it. Reusing a worn pad is a common cause of hot spots and uneven stimulation.
Checkpoints and mistakes to avoid during every session
Start at the lowest intensity and increase slowly until you feel a comfortable tingling, not pain. If the sensation is sharp or burning, turn the unit off and reposition the pad that feels wrong. Never place electrodes over the front of the neck, temples, chest across the heart, or broken skin. Do not use TENS while driving or operating machinery. If you have a pacemaker, implanted defibrillator, or are pregnant, ask your clinician before using TENS. Keep sessions to the time recommended by your device manual or clinician, and give your skin a break between sessions.
Step-by-step TENS electrode placement routine
- Clean and dry the skin: Wash with mild soap and water, rinse, and pat dry. Trim hair rather than shave if needed. Checkpoint: skin should feel dry and free of lotion or oil before any pad goes on.
- Connect leads to the device first: With the TENS unit off and intensity at zero, plug the lead wires into the device, then attach them to the electrodes. Common mistake: turning the unit on before pads are on skin, which can cause a sudden jolt.
- Place pads for your pain area: Back: straddle the spine. Knee: bracket above and below the kneecap. Shoulder: wrap front to back around the shoulder cap. Checkpoint: pads should be at least 2 inches apart and on muscle, not bone.
- Press edges and start low: Press each pad edge down firmly, then turn the unit on and raise intensity slowly until you feel gentle tingling. If one pad stings, turn off, press it down again, and restart.
- Adjust or stop if needed: If the sensation is sharp or burning, turn the unit off and reposition the pad that feels wrong. Do not push through pain. Checkpoint: comfortable tingling across the area, not a pinpoint sting.
- Remove and store pads properly: Peel pads off slowly, place them back on their plastic liner, and seal them in a bag away from heat and sunlight. Replace pads when they no longer stick or the hydrogel pulls away from the edges.
Electrodes and accessories that support consistent placement
- Starburst Electrodes | Silver TENS/NMES/FES Pads with Aloe Hydrogel, 4 per Package: Starburst Electrodes from Balego are reusable electrotherapy electrodes for compatible TENS, NMES, EMS, and FES devices, made by Uni-Patch with a patented silver Starburst current-distribution pattern, conductive carbon-film base, thick aloe hydrogel, and a soft molded pigtail connector. The silver pattern and thick hydrogel help spread current evenly across the pad, which reduces the hot spots that make you reposition pads mid-session. The pigtail connector stays put when you move, so your back, knee, or shoulder placement holds.
- VitalStim® Plus Accessories & Replacement Parts: VitalStim Plus Accessories & Replacement Parts include snap lead wires, an sEMG reference lead wire, an operator hand switch, a protective rubber sleeve, a stylus, a tabletop stand, a battery door, and a reference EMG electrode for the Chattanooga VitalStim Plus system. If your lead wires are frayed or the snap connection is loose, even perfect pad placement will feel intermittent. Replacing the lead wire is often the fastest fix when stimulation cuts in and out.
